The process of registering a death falls under the auspices of the Ministry of Home Affairs. Provided here below is the form to register a death.

Process to Legally Register a Death

The application form must be accompanied with the following documentation as evidence of the deceased’s death:

  1. A copy of the Death Notification Form from the Hospital/Health Facility where the death took place.
  2. A copy of the burial or death notification from a church minister.
  3. A Statutory Declaration signed by parents/guardian and/or an immediate relative of the deceased in the presence of a magistrate or a Commissioner for Oaths.
